How do you get yourself back on track?
Beating yourself up is not the way.  There is no positive affect from beating yourself up.  Despite its ineffectiveness, many of us use it as a way to motivate ourselves.

Instead, try being the scientist of your own life.  Step back and objectively observe what is happening.  Try to raise your awareness of what you are doing both positively and negatively.
What behaviors are helping you and what ones are hindering you.

It is the tiny changes over time that make the biggest difference.
Gisele share an example from her business and how looking more closely at the metrics that are most reflective of her business's health.
This works in business or in your personal life. 
1.  What are you trying to accomplish?
2.  What one or two behaviors are going to make the biggest improvement? 
3.  Try to state what you want as "I want to go from x to y by when.."
