[02:19] 20% of patients with multiple chronic conditions consume 65% - 70% of Medicare spending, the biggest demographic being seniors.

[02:30] Minimizing and preventing unneeded hospitalization of seniors can save a lot of money for Medicare, as well as families.

[03:30] People are healthier, and living longer than ever before, but we are not better off when it comes to chronic diseases.

[05:07] Starting from 2015, Medicare started adding payment codes to incentivize doctors to provide value-based care.
